A Reliable Geriatric Assessment and Memory Care Clinic
At Richland Medical Center, we aim to provide a comprehensive, multidisciplinary assessment of aged people with memory concerns. Leveraging our geriatric assessment clinic, we are able to educate our patients and their families and give them treatment recommendations. We do not provide primary medical care if ever the need arise, but we will directly communicate with your physician to help with your needs.
To evaluate an older person’s functional, cognitive, mental, and psychosocial abilities, we schedule two appointments for them. The first one will last for about two hours while the follow-up will only be for an hour. Follow-up visits will be scheduled as necessary.
It is crucial to have a family member or a close friend attend the first appointment with you. Your detailed history will be recorded, and the assessment will include memory testing, functional assessments, and a physical examination.
At the follow-up appointment, we will provide a summary of the assessment results, diagnoses, and treatment recommendations. These will be given and discussed with you and your family. A copy of your summary will also be handed to your physician. Other necessary information regarding dementia, dementia care, and community resources will also be provided.
